Can anyone explain to me why the State of FL requires a DNR form to be on yellow paper? Why does your doctor need to sign off on it? I would think this would be your decision.

We have living wills drawn up by our estate planners....signed and notarized by our attorneys.

All that is required is to present a copy to the treating institution and it is not on yellow paper.

I believe it is for quick and easy ID of the form. I have also been told it must be on yellow paper - that is what the EMT's look for.

THEY were wrong wrong wrong! A doc does not have to sign it. It needs to be constructed properly, signed, notarized, and the facility needs a copy - not the original. Mine and my wife's, along with our healthcare power of attorney forms were prepared by an attorney. We have access to them in the cloud by any electronic device including a smart phone. Please don't accept erroneous information at face value. Some workers don't even know what they are. If you don't have a DNR form the hospital can provide one and provide a notary and witness for your use. And it does not have to be on yellow paper.

We too had ours drawn up and notarized. EMTs recently picked up my mom and said if there was no DNR form on yellow paper signed by a doctor then they would have to work on her. They are required by law to only honor the yellow form. Maybe this applies only to EMTs.

When I'm wrong I'm wrong wrong wrong. Lt John Longacre was kind enough to respond to my inquiry regarding DNR forms. Here is his response:

"Thank you for your inquiry about the DNR form. The part that the form must be on yellow paper is correct. In order for the form to be valid it must be printed on yellow paper. We (Paramedics/EMT’s) are only required to honor the form it is printed on yellow paper.

I have included some text, internet links and a copy of the DNR form for you to review.

f. Do Not Resuscitate Order (DNRO) Form. The Department of Health (DOH), Bureau of Emergency Medical Services (EMS), recognized form (DH Form 1896) which is printed on yellow stock legal size paper (8 ½ X 14 inches). This DOH form was put into legislation to ensure the individuals’ wishes about their own death would be honored. The form is recognized across healthcare settings and also allows EMS not to resuscitate a person who has the yellow form while they transport the person should the person go into cardiac or respiratory arrest.
8. Can I print it on white paper?
No. Florida Administrative Code 64J-2.018 states specifically that the form shall be printed on yellow legal paper. The form is not valid unless it is printed on some shade of yellow legal paper. EMS providers and hospitals are not obligated to honor a form printed on white paper or any other color than yellow.

the living will does provide for resusitation and then present to the family the situation and then make the decision what to do based on the information gained. Say for example during a heart attack the victim crashes. The living will allows them to try to bring the person back to life.
If a DNR was in place there would be no attempt to revive the individual.

And yes the DNR has to be on the yellow form. The most frequent use of DNR is when someone is truly at end of life as in hospice or similar situation.

I am sure there may be variations in individual living wills, but I thought I would pass along what I learned about it.

I don't know where he got the need for legal paper. Mine was made up by the lawyer and it is on 8 1/2 X 11 yellow paper. Also, I cannot find the need for legal paper on the internet.
